Social Prescription easier

By creating an account on Clic Social, healthcare professionals can quickly identify and prescribe to their patients complementary non-medical services. They can print or send this prescription to the patient via email with the simple click of a button.

A work in progress

Clic Social is a project in the testing phase. Community resources are added to the portal constantly. Its development is based on feedback from its users and many new features are to come. We invite you to come and enrich its development by taking part now in this great movement for the overall health of the population.

The Eva Marsden Centre for Social Justice and Aging is a dynamic community organization that was founded in 1974 dedicated to restore humanity and dignity to the experience of growing older in our society. EMC works to improve all aspects of health and well-being while collectively addressing systemic issues that impact daily life. In addition to social service programs for low-income older adults, EMC is also at the forefront of innovative project development.

EMC has been a Centraide Agency for the past 40 years and continues to reflect the social justice values of our founding members. EMC contributes to the knowledge base in gerontology and social determinants of health by participating in research projects conducted at local, national, and international levels.

CIUSSS West-Central Montreal is committed to providing health care recipients with timely access to a continuum of care that meets the specific needs of each user. The territory covered by this network includes some 345,000 people who are served by more than 30 care centers. The network includes one of Montreal’s major hospitals (the Jewish General Hospital) and a wide range of complementary care centers, including three specialized hospitals, five CLSCs, two rehabilitation centers, six residential and long-term care centers, two day centers and several affiliated research centers. Treatment and care are provided by more than 11,000 staff members and some 750 physicians.

Through its links with the community and dozens of community organizations on its territory, the CIUSSS aims to direct its users and clients to the right resource, according to the identified need, the population group and the neighborhood. Through the social prescribing tool, CIUSSS fosters individual and community resilience by encouraging mutual aid and facilitating the referral process to the community or community organizations, while promoting individual autonomy and well-being.
