Social prescription is becoming increasingly popular tool for intervention in England and Australia.
It enables front-line responders—including general practitioners, nurses and other health and community workers—to quickly refer clients to a range of non-medical community resources and programs.
CIUSSS West-Central Montreal
CIUSSS West-Central Montreal is committed to providing health care recipients with timely access to a continuum of care that meets the specific needs of each user. The territory covered by this network includes some 345,000 people who are served by more than 30 care centers. The network includes one of Montreal’s major hospitals (the Jewish General Hospital) and a wide range of complementary care centers, including three specialized hospitals, five CLSCs, two rehabilitation centers, six residential and long-term care centers, two day centers and several affiliated research centers. Treatment and care are provided by more than 11,000 staff members and some 750 physicians.
Through its links with the community and dozens of community organizations on its territory, the CIUSSS aims to direct its users and clients to the right resource, according to the identified need, the population group and the neighbourhood. Through the social prescribing tool, CIUSSS fosters individual and community resilience by encouraging mutual aid and facilitating the referral process to the community or community organizations, while promoting individual autonomy and well-being.

Social prescription is a person-centred tool to prevent and promote health and well-being.
Its benefits are numerous, including:
- Less social isolation – an important determinant of health
- Fewer visits to medical clinics and emergency rooms
- Reduced levels of depression
- Better quality of life
- Increased social participation – another important determinant of health
Thanks to the Federal funding program, New Horizons for Seniors, the NDG Senior Citizens’ Council, in collaboration with CIUSSS West-Central Montreal and CLSC René Cassin, is pleased to present this database, which will enable you to practice social prescription.
