Community workers: become a social prescriber!

As community workers, we have all been there. That moment where the person you are assisting needs an extra set of hands to help them improve their current life situation. Whether it be finding housing information, issues with food security, help to fight isolation… We scavenge the internet, 211, 811 or look through our home-made booklet of resources in the hope that the resources and services we find are still active and up-to-date. We scribble on a piece of paper what we found and hand them off to the beneficiary, crossing our fingers that our info is useful and that they actually make good use of it. If we have time, we may be able to accompany them or we can call for them, but that is mostly rare.

Help is on the way

Clic Social is an online platform of available community resources that healthcare professionals and community workers can refer patients to. You can print or send this referral to the beneficiary via email with the simple click of a button. As prescribers on the platform, community workers can access complementary organizations and search for services by social determinants of health, age group and population needs. The platform includes resources that were provided through the CIUSSS West-Central Montreal, various CLSCs and community organizations themselves.
